PROOF OF INCOME: Please tell your client to bring one of these
to their ADS appointment:
- paycheck stub for last month
- income tax return
- copy of recent TANF (Temporary Assistance for Needy Families) or food stamp payments
- copy of unemployment check for last month
- copy of bank statement to verify employer direct deposit
- letter from employer on letterhead with rate of pay, hours, employer contact name & number
- letter from homeless shelter verifying residence
- copy of bank statement to verify employer direct deposit
- award letter from agency when receiving Social Security or Veteran Disability
Please arrive one hour prior to your scheduled appointment. You must also bring:
1. Driver’s license or valid ID card
2. If you have it, your Medicaid, Medicare, or private insurance card (or proof of your application for benefits)
